The Optimal Dose of Cyclosporine in Renal Transplant Recipients


Abstract
We have measured the Cyclosporine (CsA) whole blood concentration in 26 renal transplant recipients by 125I-radioimmunoassay (RIA) with specific monoclonal antiboby since 1991 and the results of analysis were as follows:
1) CsA was initially administrated from second pretransplant day with the dose of 5 mg/kg/day and maintained 10 mg/kg/day during next 7 days.
2) Monitoring of CsA whole blood levels from fifth posttransplant day and the dosage was adjusted to maintained in the range of 200~400 ng/ml in induction period and 100~200 ng/ml in maintenance period.
3) The good allograft function was observed with keeping the CsA levels in thereapeutic window.
4) Acute rejection were developed in three cases, and all was responsed by steroid pulse therapy.
5) One case was suspected as nephrotoxicity that was recovered completely with reduction of CsA dose.
According to the results, it would be needed rapid decrease of CsA dose during early posttransplant period.
